Any referral has to be through the Infectious Diseases Team. Patients are then referred either to Virtual Ward for home treatment or to the Ambulatory Care Unit for daily infusions.

Inclusion / Exclusion criteria

Once daily iv antibiotic regimens allowed only. If treatment is longer than 1 week patient should have a PICC line before discharge. Patients need to be medically fit for discharge and able to attend for daily infusion if treatment will be on Ambulatory Care Unit. Access to ACU for review is guaranteed in case of any issues when patient receives treatment at home.

Inclusion / Exclusion criteria

The OPAT ratified clinical guidelines using once daily antibiotics are:
• Skin and soft tissue infections including cellulitis
• Complex Urinary Tract Infection including pyelonephritis
• Diabetic Foot Ulcer Infection with or without osteomyelitis
• Osteomyelitis
• Septic Arthritis
• Stable patients with bacterial meningitis
• Pseudomonas Infections for patients with bronchiectasis (BD therapy)
• Rare Conditions (MDT approval)
GPs can also directly refer patients for IV antibiotics for cellulitis and cultured ESBL / E Coli UTI. Other Trusts can refer patients to the Dudley Community IV team if the patient has a Dudley GP.

Adult patients will NOT be treated within the OPAT service if they meet any of the exclusion criteria below:

• Patients with mild infection
• Children (under the age of 16 years)
• Systemically unwell
• Pyrexia ≥ 38.3 degree Celsius,
• Hypothermia < 35.0 degree Celsius • Heart Rate > 110 bpm
• Systolic BP < 90mm Hg • Altered mental state from patient’s normal condition • Respiration rate >25 breaths per minute
• Oxygen saturations <92% and not previously known to be in respiratory failure • Renal impairment (eGFR <10ml/min) • Neutropenia (require senior physician review)

Inclusion criteria
1. Antibiotic for OPAT is clinically effective for infection with appropriate antibiotic medical plan in medical notes.
2. Patient is medically, surgically and socially fit for discharge.
3. Age 16 years or above.
4. Must have daily contact with relative/carer/friend.
5. Have telephone access.
6. Consent to treatment at home or daily hospital attendance.
7. Consent to OPAT nurse assessing suitability of home conditions.
8. If the patient is to self-administer antibiotics they must

Have no significant visual impairment,
Have no significant problems affecting manual dexterity,
Consent to training to administer own IVs.
9. Suitable venous access device in place which is functional and reliable.
Exclusion criteria
1. Current intravenous drug users or patients who self harm.
2. Patients with unstable co-morbidities or insurmountable barriers to effective treatment.
3. Patients with deteriorating LFTs / U&Es.
4. Patients who have known allergies to named treatments.
5. Patients who are medically unstable and require in-patient therapy.
